Proxy Form Download & Instructions

All homes that are unable to attend the meeting should complete and sign a Proxy/Ballot form.

These forms can be returned via the following means:

1. Provide hard-copy to a neighbor who will be attending the meeting.
2. Scan and email to the FGHOA Nominations Committee at
   management@flintsgrovehoa.org.
3. Mail to Vista Management Company, 12041 Bournefield Way, Suite C, Silver Spring, MD
   20904.
4. Provide hard-copy to volunteers who will be walking the neighborhood to collect forms.

These proxies will assist us achieve a quorum of 25% of homes.
